植根啟德，照顧社區健康Community care in Kai Tak

全家醫務中心服務啟德、德朗邨、啟晴邨、麗晶花園及鄰近社區，提供普通科診症及健康服務。我們希望成為居民日常求醫、健康檢查及持續跟進的社區醫療據點。CareForAll serves Kai Tak, Tak Long Estate, Kai Ching Estate, Richland Gardens and neighbouring communities with general practice and health services. We aim to support residents with everyday consultations, health checks and ongoing follow-up.

診症以了解病人的症狀、病歷及個別需要為起點，按評估討論檢查、治療及跟進安排；有需要時轉介合適的專科或其他醫療服務。Consultations start with symptoms, medical history and individual needs. Assessment guides discussions about investigations, treatment and follow-up, with specialist or other healthcare referrals when appropriate.

兩間診所，清晰分工Our two clinic locations

晴朗商場診所現正提供普通科及相關健康服務。陳囿全醫生現於晴朗商場診所應診，亦將於煥然壹居新診所應診；預約時請確認地點及時段。Ching Long Clinic currently provides general practice and related health services. Dr Chan Yau Chuen currently consults at Ching Long and will also practise at the new De Novo Clinic. Please confirm the location and session when booking.

煥然壹居綜合醫務中心位於啟德沐翠街3號地舖，現正籌備中，預計2026年12月上旬投入服務，並設兒科及物理治療服務。個別服務的開始日期及應診安排請先向診所確認。The De Novo Integrated Medical Centre at G/F, 3 Muk Chui Street, Kai Tak is in preparation, with opening planned for early December 2026 and paediatric and physiotherapy services planned. Please confirm each service’s start date and consultation arrangements with the clinic.

從診症到預防與跟進Consultations, prevention and follow-up

服務範圍涵蓋常見疾病評估、慢性疾病管理、健康檢查、疫苗接種、婦女健康及子宮頸篩查、皮膚及過敏評估，以及按需要安排化驗與影像檢查轉介。實際項目須按醫生評估及所選診所安排。Services include assessment of common illnesses, chronic disease management, health checks, vaccinations, women’s health and cervical screening, skin and allergy assessments, and laboratory or imaging referrals when needed. Available options depend on clinical assessment and the selected clinic.

專科及復康服務方面，煥然壹居診所籌備提供兒科專科及物理治療服務；精神科預計2027年上旬推出。各項服務開始日期、醫護人員及應診安排，請先向診所確認。For specialist and rehabilitation care, De Novo Clinic is preparing paediatric specialist and physiotherapy services. Psychiatry is planned for early 2027. Please confirm each service’s start date, clinician and consultation arrangements with the clinic.

營養及臨床心理服務現可經轉介安排。如希望了解適合的服務、所需文件或預約流程，可先向診所職員查詢。Nutrition and clinical psychology services are available through referral. Our clinic staff can help with enquiries about suitable services, required documents and booking arrangements.
