慢性疾病的跟進安排因病情而異。覆診可用作討論現有治療、不適、已有紀錄，以及是否需要檢查或轉介，並非每人都需要同一套項目。Follow-up for a chronic condition is individual. A review can address treatment, symptoms, existing records and whether tests or referral are needed. The same set of tests does not suit everyone.

與醫護討論甚麼？What can I discuss with a healthcare professional?

請帶備藥物清單及已有報告，提出副作用、漏服或用藥方面的疑問。不要自行停藥或加減劑量；如有新症狀或藥物快用完，應及早向原醫護查詢。Bring medicine lists and available reports, and raise concerns about side effects, missed doses or treatment. Do not stop or alter prescribed doses yourself. Contact your clinician early about new symptoms or a low medicine supply.
